(Yaay, here's my chance to pick on Brede!)  Why, does the main character cut off his manhood?  Miss Rand would never approve, and neither would most people who understand where babies come from (and a newer way to make babies probably wouldn't come around for a few generations more).  Though I could totally respect someone losing genitalia as the result of torture (like most defendants at Nuremberg Trials did) or in an industrial accident of some sort, and then switching genders to make the best of things.  You can't expect everybody to relate to doing that sort of thing voluntarily, or else the Demographic Winter would wipe out human civilization within our lifetimes.  Like is written in The Transsexual Bible, "hysteria is impossible without an audience".
